Treatment overview

Hair transplant surgery at Olive Skin & Aesthetics is a permanent solution for pattern baldness and hair thinning. Our team utilises advanced FUE and FUT techniques to restore natural-looking hair density with minimal scarring.

The goal of a hair transplant is to redistribute healthy, genetically resistant hair follicles to areas of thinning or baldness. Patients can expect a fuller hairline and improved self-confidence with results that continue to mature over 12โ€“18 months.

  • Adults with androgenetic alopecia (male or female pattern hair loss)
  • Individuals with stable hair loss who have sufficient donor hair density
  • Patients seeking a permanent, natural-looking restoration without ongoing medication
  • Those with hair loss resulting from trauma, burns, or previous surgical scars
  • Non-smokers in good general health with realistic expectations about coverage

The procedure is performed under local anaesthesia and typically takes 7โ€“8 hours depending on the number of grafts required. Individual follicular units are extracted from the donor zone at the back of the scalp using a micro-punch instrument.

Individual follicular unit or strip extraction is done from the donor zone. The extracted hair grafts are prepared followed by the actual process of implantation. We employ a Choi implanter pen for precise angle, depth, and direction control, minimising handling trauma to grafts and allowing denser packing in the recipient area for a completely natural growth pattern.

  • Day 1โ€“2: Mild swelling and pinpoint crusting at extraction and implantation sites; pain managed with oral analgesics.
  • Day 3โ€“5: Swelling may migrate to the forehead; sleep with head elevated and avoid touching grafts.
  • Week 1โ€“2: Crusts shed naturally; hair wash permitted as advised.
  • Week 3โ€“4: Transplanted hairs enter a shedding phase (shock loss) โ€” this is normal and temporary.
  • Month 3โ€“12: New growth begins around month 3 and progressively thickens; final density visible by 12โ€“18 months.

Frequently Asked Questions

What is the difference between FUE (Follicular Unit Extraction) and FUT (Follicular Unit Transplantation)?

FUT (Strip method) involves surgically excising a narrow strip of scalp from the permanent donor zone (occipital area), dissecting it under microscopes into individual follicular units, then suturing the donor site โ€” leaving a single linear scar hidden within the hair. It allows the highest graft yields per session (up to 4,000+ grafts) with excellent follicle transection rates. FUE extracts individual follicular unit grafts one by one using a micro-punch (0.7โ€“1.0 mm) directly from the scalp, leaving no linear scar โ€” ideal for short haircuts and patients who prefer not to have a strip scar. Modern Sapphire FUE (using jewel-tipped blades) and DHI (Direct Hair Implantation with Choi pens) are advanced FUE evolutions offering higher graft survival and more natural angles.

How many grafts will I need, and how is this determined?

Graft requirements are determined by: the Norwood-Hamilton scale classification of your baldness pattern, the density deficit (recipient area size in cmยฒ), the desired final density (typically 40 to 60 grafts per cmยฒ for a natural look), and your available donor density and donor area surface. A typical session for Grade III to IV baldness requires 1,500 to 2,500 grafts. Grade V to VI baldness may require 3,000 to 4,500 grafts over one or two sessions. A trichoscopy examination and digital follicular density mapping are performed at consultation to calculate exact graft requirements with precision.

What is "shock loss" (telogen effluvium) and should I be concerned?

Shock loss (effluvium) refers to the temporary shedding of transplanted hair follicles 2 to 6 weeks after a hair transplant โ€” a normal physiological response as follicles enter telogen (resting phase) following the trauma of extraction, preservation, and implantation. Simultaneously, native hair in and around the recipient zone may also shed temporarily due to surgical disruption (reactive effluvium). Both the transplanted and shed native hair re-enter anagen (growth phase) and regrow between months 3 and 6. By month 9 to 12, approximately 90 to 95% of transplanted grafts produce permanent, growing hairs.

Will the donor area on the back of my head look visibly thin or patchy after graft harvesting?

When FUE harvesting is performed correctly, grafts are extracted in a dispersed, randomized pattern across the safe donor zone (occipital and parietal scalp) โ€” never in one concentrated area. Punch sizes of 0.8 to 0.9 mm leave tiny circular marks that are invisible when hair is more than 1 cm long. In FUT, the linear strip scar sits within the permanent hair zone and is covered by surrounding hair when grown to 1.5 to 2 cm. However, over-harvesting beyond the safe donor zone (which is DHT-resistant) can deplete the donor area permanently. Conservative harvesting โ€” guided by donor density assessment โ€” is mandatory at Olive.

Can a hair transplant repair a previous poorly performed transplant?

Yes โ€” repair transplantation is one of the most technically demanding areas of hair restoration surgery. Common deformities from previous transplants include: doll-plug appearance (large round grafts from older techniques), an unnatural "cornrow" straight hairline, visible donor scarring, and poor frontal density despite excessive graft placement. Repair strategies include: follicular unit refinement of plugs using FUE micro-dissection, camouflage of a linear FUT scar with FUE grafts implanted directly into the scar, hairline redesign with micrografts in natural irregularized patterns, and strategic redistribution of healthy grafts to correct density deficits.

Will transplanted hair be permanent, and will I continue losing my native hair?

Hair follicles transplanted from the permanent donor zone (occipital scalp) carry the DHT-resistant genetic programming of their origin โ€” meaning they are inherently resistant to the androgenetic alopecia (male/female pattern baldness) that caused the original hair loss. This genetic permanence is maintained even when transplanted to a bald area (donor dominance principle). However, your original native non-transplanted hair in androgenetic zones will continue to follow its natural miniaturization and loss trajectory unless managed with medical therapy (finasteride and/or minoxidil). Long-term concurrent medical treatment is strongly recommended to preserve native hair.

When will I see the final result, and what is the growth timeline?

The hair transplant growth timeline follows a predictable sequence: Days 1โ€“14: red dots and crusting at recipient sites resolve; Weeks 2โ€“6: transplanted hairs shed (shock loss โ€” normal); Months 2โ€“4: new hair stubble begins growing from surviving follicles; Months 4โ€“6: visible early density, still thin and soft; Months 6โ€“9: progressive thickening and caliber increase; Months 9โ€“12: approximately 80 to 90% of final result visible; Month 12โ€“18: full caliber, texture, and density of final result achieved. Most patients require 12 to 18 months to appreciate their complete, definitive cosmetic outcome.
