Treatment overview

A 360 Body Lift โ€” also known as a circumferential body lift or belt lipectomy โ€” is a transformative procedure that addresses sagging skin and loss of contour around the entire circumference of the torso. At Olive Skin & Aesthetics, We perform this procedure with meticulous attention to scar placement and tissue handling, ensuring the most natural And proportionate outcome.

The procedure simultaneously lifts and tightens the abdomen, flanks, lower back, buttocks, and outer thighs in a single circumferential excision โ€” making it the most comprehensive body reshaping option available. It is most commonly indicated for patients who have lost a significant amount of weight and are left with extensive excess skin that causes functional discomfort, hygiene concerns, and profound dissatisfaction with their body image.

The results are dramatic and long-lasting: a smooth, lifted silhouette that extends all the way around the body, restoring proportion and confidence that significant weight loss alone cannot achieve.

  • Adults who have achieved significant weight loss โ€” either through bariatric surgery or lifestyle changes โ€” and are left with substantial excess skin around the torso
  • Patients who have maintained a stable weight for a minimum of 12โ€“18 months following their weight loss
  • Individuals with good nutritional status, as extensive surgery requires robust healing capacity
  • Non-smokers, or patients who have completely stopped smoking for at least 6 weeks before and after surgery
  • Those with realistic expectations about scarring โ€” a circumferential scar around the lower torso is an inherent and unavoidable part of this procedure
  • Patients who have completed family planning, as pregnancy after a 360 lift is strongly discouraged
  • Adults psychologically prepared for an extended recovery period and the lifestyle adjustments it requires

The 360 Body Lift is performed under general anaesthesia and typically takes 4โ€“6 hours, requiring an inpatient stay of 2โ€“3 nights. The procedure is planned meticulously in the pre-operative assessment, with marking the skin in a standing position to ensure optimal scar placement and tissue redistribution.

The surgery involves removing a circumferential band of excess skin and fat from around the lower torso. The abdominal component addresses the lower abdomen and flanks; the posterior component simultaneously lifts the buttocks, outer thighs, and lower back. Liposuction may be used to further refine areas above and below the excision zone where necessary.

Closure is performed in multiple tension-relieving layers to distribute tension evenly, minimise scar widening, and support the newly elevated tissues. Drains are placed and a compression garment is applied before the patient is woken from anaesthesia.

  • Days 1โ€“3 (hospital): Close monitoring, IV fluids, and pain management. Drains in place. Light mobility โ€” short walks โ€” begun on day 1 to reduce clot risk.
  • Week 1โ€“2: Discharge home with drains. Rest with assistance required. Compression garment worn continuously. No driving or strenuous activity.
  • Weeks 3โ€“6: Drains removed. Swelling and bruising subside progressively. Light daily activities resume. Avoid lifting anything over 5 kg.
  • Months 2โ€“3: Most swelling resolved. Contour improvements clearly visible. Light exercise permitted. Return to most desk-based work.
  • Months 4โ€“6: Scar matures and softens. Full result emerges. Gym and vigorous exercise resumed with surgical clearance.
  • Month 6+: Final result. Scars continue to fade for 12โ€“18 months. Annual follow-up recommended.

Frequently Asked Questions

What exactly is a 360 Body Lift, and who is the ideal candidate?

A 360 (circumferential) Body Lift is a single, comprehensive operation that removes the belt of excess, sagging skin and fat encircling the entire lower trunk โ€” the abdomen, flanks, lower back, and upper buttocks โ€” through a single continuous incision placed circumferentially around the waistline. It is the definitive procedure for patients who have undergone massive weight loss (bariatric surgery or diet and exercise) and are left with pannus (apron-like skin folds) hanging circumferentially. Candidates should be weight-stable for 12 to 18 months, at or near goal weight, non-smoking, nutritionally optimized, and medically cleared.

Where is the circumferential scar placed, and how is it managed?

The incision encircles the entire lower trunk like a belt, positioned at the level of the bikini or underwear line โ€” approximately at the level of the anterior superior iliac spine anteriorly, tracing over the iliac crest laterally, and sitting within the natural gluteal crease and lower back posteriorly. In loose clothing or swimwear, the scar is entirely concealed. The long scar fades progressively over 12 to 24 months. Silicone gel sheeting and scar massage beginning at 6 weeks, combined with sun protection, are essential for optimal scar maturation.

How must I prepare nutritionally for a 360 Body Lift after bariatric surgery?

After bariatric surgery, significant deficiencies in protein, iron, vitamin B12, vitamin D, calcium, and zinc are common due to altered absorption. These deficiencies impair wound healing and increase infection risk. Pre-operative optimization typically requires 3 to 6 months of supervised nutritional rehabilitation: protein intake raised to 80 to 100 g/day, all specific deficiencies corrected to within normal laboratory ranges, hemoglobin raised above 12 g/dL, albumin above 3.5 g/dL, and pre-albumin above 15 mg/dL. Your plastic surgeon works closely with the bariatric team and a dietitian throughout this preparation.

Is the buttock lifted and shaped during a 360 Body Lift?

Yes โ€” a major advantage of the 360 circumferential body lift is that the posterior component simultaneously elevates and reshapes the ptotic (drooping) buttock. As excess posterior skin is excised and the lower back skin is elevated, the gluteal tissue is autoaugmented (lifted and projected forward) using internal suspension sutures โ€” the "autoaugmentation gluteoplasty" technique. This avoids the need for implants or fat transfer to improve buttock projection while simultaneously lifting and defining the gluteal crease, creating a more youthful, rounded buttock contour.

How many surgical drains are placed, and how long are they required?

Due to the extensive area of skin detachment in a 360 body lift, 3 to 4 closed-suction drains (Jackson-Pratt or Blake) are routinely placed โ€” anteriorly beneath the abdominal flap, and posteriorly on each side of the lower back. Patients measure and record daily drain output at home. Drains are removed sequentially when output consistently falls below 25 to 30 mL per 24 hours. Most patients require their anterior drains for 7 to 10 days and posterior drains for 10 to 14 days. Progressive tension sutures (internal quilting) minimize dead space and allow earlier drain removal.

When will I be able to walk upright, and what are the activity milestones?

Due to simultaneous anterior abdominal and posterior truncal skin tension, patients walk in a slightly hunched position (15 to 20 degrees flexion) for the first 7 to 14 days until incisional tension naturally relaxes. Fully upright posture is typically achieved by day 10 to 14. A recliner chair or wedge pillow system is ideal for home rest during the first week. Return to desk work is typically at 2 to 3 weeks; light walking at 3 weeks; and full activity, including exercise, at 6 to 8 weeks. Circumferential compression garments are worn continuously for 6 to 8 weeks.
