Gynecomastia surgery at Olive Skin & Aesthetics treats enlarged male breast tissue to restore a flat, masculine chest contour. We combine liposuction with glandular excision techniques tailored to each patient’s anatomy and grade of gynecomastia.

The procedure eliminates excess breast gland and fatty tissue, flattening the chest and creating a firmer, more defined pectoral appearance. Patients regain confidence to wear fitted clothing and participate in activities without self-consciousness.

  • Males with persistent breast enlargement unresponsive to weight management or hormonal treatment
  • Patients in good overall health with stable hormone levels
  • Individuals whose breast development has stabilised (typically age 18 or older)
  • Non-smokers and non-anabolic-steroid users
  • Those experiencing psychological distress or social withdrawal due to enlarged breast tissue

Gynecomastia surgery is performed under general anaesthesia and takes approximately 1.5โ€“2 hours. The approach depends on the composition of excess tissueโ€”liposuction alone for fatty gynecomastia, or combined with peri-areolar gland excision for fibrous or mixed types.

We use VASER-assisted liposuction to address the fatty component with precision, followed by direct excision of dense glandular tissue through a small incision at the areolar border. This combination ensures a smooth, flat result without contour irregularities.

  • Days 1โ€“3: Chest tightness and moderate soreness; compression vest worn continuously; arms rested at sides.
  • Days 4โ€“7: Most patients return to desk work; swelling and bruising begin to subside; avoid lifting above shoulder height.
  • Weeks 2โ€“3: Compression vest worn during daytime; light lower body exercise permitted; chest contour improving.
  • Week 4: Full recovery reached; upper body activity gradually reintroduced; compression vest discontinued.
  • Months 3โ€“6: Final chest contour visible; scars at areolar edge fade to near-invisible lines.

Frequently Asked Questions

What is the difference between true glandular gynecomastia and pseudogynecomastia?

True gynecomastia is characterized by the benign proliferation of firm, rubbery glandular breast tissue directly behind the nipple-areolar complex, caused by an imbalance between estrogen and testosterone. Pseudogynecomastia (or lipomastia) consists purely of excess fatty tissue distributed across the chest, typically associated with weight gain. Many men present with a mixed form containing both dense glandular tissue and diffuse subcutaneous fat. During your consultation, physical examination and ultrasound evaluate the proportion of gland versus fat to customize the surgical approach.

Why don't diet, chest workouts, and pushups eliminate my gynecomastia?

While rigorous workouts and calorie restriction burn fat and build pectoral muscle, they cannot burn glandular tissue. Glandular breast tissue is fibrous, hormonally responsive tissue that does not respond to exercise. In fact, developing larger pectoral muscles often pushes the stubborn glandular mass further outward, making the puffy nipple appearance more pronounced. Permanent resolution requires surgical excision of the gland itself.

How is gynecomastia surgery performed at Olive?

At Olive, we utilize a dual-technique approach to ensure a sculpted, flat masculine chest wall: First, ultrasound or micro-cannula liposuction sculpts the surrounding pectoral and lateral chest fat to blend the chest smoothly into the ribs and latissimus dorsi muscle. Second, a semi-circular Webster incision along the lower border of the areola allows direct excision of the firm glandular tissue. A thin, millimeter layer of tissue is preserved beneath the nipple to prevent an unnatural sunken crater or saucer deformity.

Will there be visible scars on my chest after gynecomastia surgery?

Scars are intentionally designed to be virtually imperceptible. The primary incision is placed right along the lower border of the areola (the periareolar junction), where natural color transitions from darker areolar skin to normal chest skin disguise the scar. The tiny 3โ€“4 mm liposuction entry ports are hidden within the armpit fold and the lateral chest crease. With proper scar care, these marks fade over 6 to 12 months, allowing you to comfortably take off your shirt at the gym or pool with total confidence.

Can gynecomastia return after surgery?

For the vast majority of men, gynecomastia surgery provides permanent, lifelong results because the excised glandular tissue cannot grow back. However, recurrence is possible under specific external triggers: significant weight gain (which enlarges remaining fat cells), the use of anabolic steroids or testosterone boosters in bodybuilding, certain prescription medications (anti-androgens, certain antidepressants, or ulcer drugs), or excessive recreational marijuana and alcohol consumption. Maintaining a healthy lifestyle protects your sculpted chest permanently.

How long must I wear the chest compression vest, and what does it do?

You will wear a custom medical compression vest continuously (day and night) for the first 3 to 4 weeks, and optionally during daytime activities for another 2 weeks. The vest is critical: it keeps the skin tightly compressed against the pectoral muscles to eliminate empty dead space, prevents fluid pooling (seroma or hematoma), drastically reduces post-surgical swelling, and facilitates the skin retraction necessary for a crisp, flat masculine contour.

When can I resume workouts, bench presses, and heavy lifting?

You can resume light walking and office work within 2 to 3 days. Light cardio (such as stationary cycling) can be reintroduced at 2 weeks. However, direct chest exercises, bench presses, pushups, heavy lifting (>10 kg), and contact sports must be paused for 4 to 6 weeks to avoid straining pectoral attachments, raising chest blood pressure, or causing delayed internal bleeding.
